Instruction Field
This field displays instructions to users. Instructions appear as gray, italicized text and will not appear in reports.

Section Field
Inserts a break in the form to create a new section. These sections are most often used to differentiate between groups of tasks (e.g., Interior vs. Exterior questions).

Yes/No Question Field
Asks the user a question requiring a yes or no answer. You can also choose to allow N/A as an option. With yes/no questions, the pass/fail option is always required—you can decide the pass/fail rating however you like.

Text Field
Requires the person filling out the form to add their own text response.

Multiple Choice Question Field
Ideal when you need the user to select the best answer from various options. Type the question in the Title box, then enter the choices in the Options box, separating each choice with a comma.

Number Field
Requires a numeric answer from the user.

Checkbox Field
Requires the user to check a box for the answer. This could be a checklist or asking if a question/product exists or not.

Automatically captures the date. You can also capture the time by clicking on the "Date" checkbox option.

Rating Field
Asks the user to rate an aspect of your business on a predetermined scale of your choice (e.g., 1-10, 1-100).

Asks the user to enter an email address.

Stopwatch Field
Lets a user start and stop a timer, as well as enter time manually if necessary.
Normal Stopwatch: Performs exactly as expected—after the user presses the 'Stopwatch' button, a stopwatch begins. When done, they hit stop and the time is saved.
Manual Stopwatch: Acts more like a time selector. Rather than starting and stopping the stopwatch, users can enter the amount of time something took by setting the time manually. (Example: "How long until you were greeted once entering the building?")

Photo Field
Asks the user to take a photo. Users can also add notes describing the photo when necessary.

Signature Field
Allows a user to capture a signature while out in the field.

Barcode Field
Allows a user to capture a barcode or QR code while in the field. The submitter can scan a barcode or QR code with their mobile device's camera, or manually enter using the keyboard. The URL/code will then be filled into the field.

Video Field
Allows a user to take a video and attach it to their submission. Video can be added from a mobile device gallery or taken on the fly using the device's camera. Users can also add notes to the video when necessary.

Document Field
Asks users to upload/attach PDF, DOC, DOCX,CSV, XLS or XLSX files. Documents can be added from a user's gallery or drive where files are saved. It can be 25MB per file (1 file per Document Field). You can ONLY add images and documents

Location Field
Asks a user to select from a list of locations uploaded to the company account. If the user's location-sharing is enabled, Zenput will automatically look for the nearest location using GPS coordinates. If the closest location doesn't automatically load, the submitter can start typing the location's name and Zenput will filter against all company locations.

Formula Field
Gives a score based on how questions in the form are answered. A formula is calculated using SUM (addition), SUB (subtraction), AVG (average), or PCT (percentage). When setting up a formula, select the calculation method and choose which questions to include. For multiple-choice and yes/no questions, you may also need to assign values to specific answers for score calculation.

Temperature Field
Captures temperatures at your location by allowing users to enter temperature values. When setting up temperature questions, you can "Enable the Pass/Fail range" to a predetermined or custom temperature range. You can also check "Enable Dependent Questions" to display follow-up questions when the response is a Pass, Fail, Pass or Fail, or Neutral.

Sensor Temperature Field
For Zenput Temperature Monitoring customers, this field automatically pulls readings from sensors directly into forms. You can include readings from ALL sensors or limit to specific sensor placements.
This field shows when the last reading was taken, helps prevent pencil whipping at the restaurant level, and draws attention to any offline sensors and battery levels so you can take necessary next steps.

Oil Quality Field
For customers who use frying oil, this field captures TPM (Total Polar Materials) readings to ensure timely oil changes and consistent food quality. Using the Oil Quality field increases accountability at the store level—helping maintain food quality while also saving costs by avoiding premature oil changes based on guesswork.

Mobile View & TPM Ranges
Users enter the oil's TPM reading manually (unless using an integrated Testo probe). Based on the answer, the field displays a response managed by the Pass/Fail range settings:

24% and below = Pass (Oil quality is good) |
25–27% = Warning (Change oil when next possible) |
28% and above = Fail (Change oil immediately) |
