Forms: Pass/Fail ratings

Note: You need to have owner or admin permission to set up pass/fail ratings in forms.
Pass/Fail ratings allow Zenput to understand the business impact of specific answers in your forms. When enabled, Zenput can automatically perform actions based on whether a question passes or fails, trigger dependent follow-up questions, and provide visual feedback to submitters. This feature supports enhanced reporting and alerts, helping you highlight positive outcomes and identify areas for improvement across locations and users.

Set Up Pass/Fail for Yes/No Questions

With yes/no questions, the pass/fail option is always required. You can configure which answer (Yes or No) constitutes a pass or fail based on your business logic.

  1. From the Dashboard, click on 'Forms' and select the form you would like to edit.

  1. Select the yes/no question you would like to edit. Configure the pass/fail rating based on your requirements.

    Example: "Is there an expired product on the shelf?" — In this case, Yes = Fail and No = Pass.

Set Up Pass/Fail for Temperature Questions

Temperature questions allow you to define acceptable ranges that determine pass or fail outcomes.

  1. From the Dashboard, click on 'Forms' and select the form you would like to edit. Select the temperature question and check the box 'Enable Pass/Fail Range'.Enable Pass/Fail Range
  1. Click the drop-down arrow next to 'Select preset range'. Choose from FDA-guideline preset temperatures or enter custom temperature values. Preset temperatures can be edited after selection to suit your needs.

  1. Define your pass, fail, or neutral ranges as needed, then click 'Publish' to save.Temperature range configuration

Configure Dependent Questions

Dependent questions allow you to trigger follow-up questions based on pass/fail outcomes, helping you gather additional context when needed.

  1. After setting up pass/fail, click 'Enable Dependent Questions' to configure follow-up questions.

    Example: "Are the umbrellas in good working condition?" When the answer is No (fail), a follow-up checkbox appears stating "Umbrella has been fixed and replaced" with a photo field for proof.Dependent questions setup
  1. For temperature questions, you can configure dependent questions to display when the answer is a pass, fail, both, or neutral.

    Example: If ice water temperature fails, the question "Why is it so low?" and a Photo field will appear.

Understanding Visual Pass/Fail Indicators

Zenput provides visual feedback to alert submitters whether an answer is a pass or fail using color-coded indicators.

Note: Admins can ONLY see the red and green color indicators on the emailed version, the PDF, and the share link version — NOT from the web-app view of submissions.

Yes/No Questions

Pass (Green): The selected answer turns green when it meets the pass criteria. Note: "No" can also be configured as a pass depending on your form logic.

Fail (Red): The selected answer turns red when it meets the fail criteria.

Temperature Questions

Pass (Green): When the temperature is within the predetermined acceptable range, the field displays green.

Fail (Red): When the temperature is outside the acceptable range, the field displays red.
